Summer Night Lights Teams Commitment Form

WELCOME

This is the official form to sign up your baseball or softball team to participate in the Summer Night Lights League.

ALL TEAMS are welcomed to apply:
- If you are a DYCD Funded program, please be prepared to identify which program.
- If you are an NYPD Blue Chips program, please be prepared to identify which precinct.
- If you are not connected to a DYCD Funded program, please be prepared to describe your program.
- If you do not have a team, but have some youth interested in applying, you are in the wrong link. Please go back to the landing page and sign up your youth in the individual interest form.
